=== Effects on longitudinal bone growth === Cilostazol has been investigated in preclinical models as a stimulator of endochondral bone growth. PDE3 is expressed in proliferating chondrocytes of the growth plate, and that Pde3b-knockout mice show enlargement of the tibia and other long bones. In embryonic mouse metatarsal explant cultures, cilostazol at 10 μM increased longitudinal outgrowth over four days, expanding the round and columnar chondrocyte zones and increasing alcian-blue-stained extracellular matrix; the PDE3 inhibitors milrinone, anagrelide and olprinone produced comparable effects, while inhibitors selective for PDE2 or PDE10 did not. When three-week-old C57BL/6 mice received daily intraperitoneal injections of cilostazol at 10 mg/kg for four weeks, naso-anal body length was significantly greater than in vehicle-treated controls, and a similar growth-promoting effect was observed in Fgfr3-transgenic mice, a model of achondroplasia. It became clear that IRP was a pathologic condition associated with obstructed defecation syndrome. For a period after 2002, the minimally invasive trans-anal approach known as stapled trans-anal rectal resection (STARR) was popular for treating obstructed defecation syndrome and related conditions such as IRP. However, this procedure has inconsistent results and tends to cause problems with bowel urgency and urge fecal incontinence. Ventral (anterior) rectopexy was developed in 2004. This procedure avoids lateral or posterior dissection of the rectum, except at the level of the sacral promontory in order to fix the mesh. Ventral rectopexy was first used for external rectal prolapse, and the improvement in constipation lead to surgeons using the same procedure for IRP. Laparoscopic approach has made surgery for IRP safer with less complications and faster healing. Ventral rectopexy appears to have more favorable results and less risk of recurrence for IRP compared to older procedures, and is currently the preferred surgical procedure for treatment of IRP.
